Home & Health Care Management is the oldest home health care agency
in Northern California. Our agency specializes in delivering
comprehensive in-home care and case management for older adults,
children, and those living with disabilities across many counties.
We use a matrix of Medi-Cal, Medicare and targeted health programs
to find solutions that work for our clients. Overview of the Role
We want to hire bachelors prepared Social Workers (or another
related field such as Gerontology) who will work with a Care
Management Team (CMT) consisting of Masters or Bachelors prepared
Social Worker and Registered Nurse team to provide comprehensive
case management services to target population. Who You Are: You are
a Master's or Bachelors Prepared Social Worker (BSW) passionate
about making a difference in the lives of individuals. You are
detail oriented, computer literate, and want to work as part of a
management team supervising the care of clients who are receiving
home and community-based services. You are good at problem solving,
knowledgeable about community resources, and enjoy helping clients
solve social determinants of health care problems. You want a
workplace that values the significant differences Social Workers
can make to the health and welfare of clients. You like working
8am-5pm on weekdays with no weekend work. Responsibilities: Assists
the team with finding community resources (i.e., housing; medical
equipment and supplies; primary care physician and specialists;
essential items like food, clothing, and household goods; medical
transportation; and caregivers, etc.) needed to meet the
participants needs. Engages in advocacy efforts to ensure
participants receive timely medical services to remain safe in the
community Responds timely to questions and enquiries from CMT,
participants, and other entities for smooth patient care
coordination Provides education to participants so they can better
understand the benefits of services and community supports. Assists
participants with applications (i.e., Medi-Cal; IHSS; Calfresh; and
Regional Center, etc.). Explains eligibility requirements,
coordinates transportation for participants and other basic needs
of support Document all consumer interaction and maintain timely
reporting and documentation requirements Observes participants
behavior; reports significant changes to supervisor or team;
documents appropriately Daily case conference with CMT about shared
participants Performs other related duties as assigned. Essential
Requirements: Master's degree or Bachelors degree in Social Work or
another related field Skilled in operating a personal computer
utilizing a variety of standard software Valid California Drivers
License and eligible to be insured under our liability policy with
a clean DMV report Your own vehicle and proof of current auto
insurance CPR certificate required within 30 days of hire Preferred
Skills, Knowledge and Certifications Knowledge of basic problems
and needs of people diagnosed with a physical disability or
emotional disturbance Knowledge of community and supportive
services Skilled in assessing and prioritizing multiple tasks,
projects, and demands Skilled in working within deadlines to
complete projects and assignments Skilled in establishing and
maintaining effective working relations with co-workers and other
community providers Experience with electronic medical records
Location and Service Region: This is not a remote role, and you are
required to work from our Sacramento office. You will need your own
car and be able to travel to clients homes in Sacramento County and
the greater surrounding area. Work is performed in a community and
office environment. PPE is provided to all employees. Transporting
clients with personal vehicle is prohibited Schedule: HHCM is
recruiting for Full-time and Part-time positions! The full-time
role is 40 hours per week, from Monday through Friday from 8am to
5pm with an hour for lunch. No weekend work. The part-time role
will vary working anywhere from 8-29 hours a week. We are seeking
part-time, temporary team members to assist with backlog tasks and
help us meet current project demands. Currently 20% of the time is
related to direct home visits, 80% of the time is telehealth video
visits and or calls and documentation in the office. Physical
Requirements: Job offer is conditional upon successfully passing a
medical exam that tests amongst other things the ability to move,
visual acuity, and hearing acuity. Physical requirement for this
role specifically includes Sits, stands, bends, lifts, walks, and
moves intermittently during working hours Able to lift to 35 pounds
without assistance Able to drive intermittently Compensation: Pay
range is $22 to $30 per hour. Exact compensation may vary based on
skills and experience. Benefits You will be reimbursed for the
mileage you travel from the office to participants homes. You will
receive a monthly phone stipend. On the first of the month
following 2 months of employment you will be eligible for health
insurance, dental insurance, vision insurance, life insurance, long
term disability insurance as well as Flexible Spending Account
(FSA). We offer 401(k) matching up to 4% after 1 year of employment
and 1,000 hours worked. Leave accrues starting on your first day of
employment, and you can take paid vacation, sick and holiday leave
