Assists the provider (s) and care teams in the management and
coordination of Patient Care, with an emphases on prevention and
health maintenance. Communicates effectively with providers and
care team members about patient's clinical condition, including
results of diagnostic studies and symptomatology. Provides care
that is mission oriented, meets the psychosocial, physical and
general aspects of delivering patient center care regardless of
racial, ethnic and economic status; meets the communication needs
of patient and family to foster a positive patient/family
experience; maintains patient confidentiality at all times,
provides care that reflects initiative and responsibility
indicative of professional expectations. Performs different tasks
that increase efficiency to help patients quickly while maintaining
quality standards under the supervision of a Health Center
Supervisor and Lead Clinician. Maintains infection control and
safety requirements, office policies, procedures and standards.
Follows organizational guidelines for quality and safety as well as
adhering to all Patient Centered Health Homes (PCHH) processes and
workflows. Participates in the departments continuous quality
improvement activities and maintain a good working relationship
with all members of the team.
High School Diploma or equivalent.
Medical Assistant training/certification preferred OR an equivalent
combination of education and experience sufficient to successfully
perform the essential duties of the job.
Current CPR or CPR/AED certification.
Communicate clearly and concisely, both verbally and in writing and
demonstrate a high level of listening skills.
Excellent customer service and interpersonal skills.
Knowledge of patient care, addiction treatment and mental health
Knowledge of Community resources including Medicare, Medi-Cal,
Supplemental Security Income (SSI) and the county's General
Demonstrated commitment to the provision of services for the
underserved and sensitivity working with a variety of people from
low-income populations, with diverse educational, lifestyle,
ethnic, and cultural origins; disabled, homeless, substance users,
HIV (AIDS) infected, and/or physiologically impaired.

Assist and support the Center/Department/Program to meet standards

Must have valid driver's license and insurable driving record.
Perform medical assistant duties, preparing rooms, rooming
patients, record medical histories, vital signs and performing
venipuncture and immunizations.
Assist the medical team including, and not limited to the Health
Care Team, physicians, family nurse practitioners, and physician
assistants, Health Educators and Integrated Care Coordinators.
Monitor Patient Centered Health Home goals.
Encourage patient's wellbeing through health promotion.
Refer patients to primary healthcare team, as appropriate.
Keep accurate records for reporting to various funding and

Travel when necessary to support operational needs.
Work one weekend per month as needed to support operational

Physical Demands and Work Environment
The work environment is characteristic of a medical clinic
environment. The physical demands described here are representative
of those that must be met by an employee to successfully perform
the essential functions of this job. While performing the duties of
this job, the employee is frequently required to sit; use hands to
handle, or feel. The employee is also required to stand; walk; and
reach with hands and arms. The employee must occasionally lift
and/or move up to 20 pounds. Specific vision abilities required by
this job include close vision, distance vision, and the ability to
adjust focus. The employee must also possess hearing and speech to
communicate in person and over the phone. The noise level in the
work environment is usually quiet.
The employee may be in contact with individuals and families in
crisis who may be ill, using substances and/or not attentive to
personal health and safety for themselves or their homes. The
employee may experience a number of unpleasant sensory demands
associated with the client's use of alcohol and drugs, and the lack
of personal care. The employee may also be exposed to bodily fluids
(blood, urine) and hazardous chemicals.
