POSITION TITLE: Mobile Assessment Registered NurseREPORTS TO
(TITLE): Director of Assessment and ReferralDESCRIPTION OF
POSITION:Responsible for providing clinical behavioral health
assessment and evaluation services in the community to determine
levels of service needed of prospective patients to ensure referral
to the service or resource that best addresses the patients
needs.KEY RESPONSIBILITIES:Responsible for providing clinical
assessment and evaluation services to patients in the community to
determine levels of service needed, as well as coordination of
referrals to a community resource or behavioral healthcare
placement if necessary.Consults with Intake department manager,
CNO, and CEO about the acceptability of admissions.Represents and
distributes information about Santa Rosa Behavioral Healthcare
Hospital services to community healthcare entities in a positive
educational manner.Current on referral sources and interacts
effectively with patients, peers and other staff on appropriate
patient treatment options.Understands and communicates the disease
pathology associated with behavioral conditions and their models of
treatment.Facilitates the patient's access to the appropriate level
of care and setting.Establishes and maintains effective working
relationships with physicians, nursing staff and health
professional affiliates in community areas assigned.Demonstrates
strong skills in assessing client's crisis and clinical
indicators.Prepares written report of assessment results in a
timely manner (immediately following completed assessment) and
includes recommendations for the level of care and treatment
needs.Makes appropriate interventions during assessments, and
crisis situations with clients, family/friends and other
professional staff.Assesses need for involuntary hospitalization
evaluation and contacts county crisis team, law enforcement or 5150
certified AVDMH employee as needed to ensure the safety of client
and others.Provides pertinent clinical information to physician and
Intake staff who will be reviewing the patient, to ensure a smooth
transition for the patient.Maintains knowledge of the developmental
tasks and sociological theories of pertinent age groups.Maintains
knowledge of legal guidelines pertaining to EMTALA/COBRA and
abuse/neglect reporting.Stays abreast of organization's programs
and services.Participates in activities which enhance professional
growth and development.Upholds the Organization's ethics and
customer service standards.This job description is not intended to

Healthcare Hospital.RequirementsPOSITION Requirements:Knowledge and
Experience:Current California State license as a Registered
Nurse.Current CPR (Cardiopulmonary Resuscitation) certification (or
obtained within 30 days of hire).Current CPI (Crisis Prevention
Institute) certification (or obtained within the 30 days of
hire).Valid California Drivers LicenseValid Car
InsuranceFamiliarity with continuum of care and clinical
terminologyPrevious Mobile Assessment experience preferredSkills
and Abilities:Maintains confidentiality of patients at all
timesKnowledgeable of patient rights and laws pertaining to mental
